Job summary

Amedisys, part of the Optum family, is seeking a dedicated Physical Therapist to provide in-home PT services. The PT will work with patients and families to promote mobility, restore function, and improve safety and independence in the home setting.

Responsibilities include delivering skilled interventions, reexaminations every 30 days, discharge planning, and collaborating with an interdisciplinary care team to prevent hospitalizations and enhance quality of life in patients' homes.

Qualifications

• Graduate from an accredited physical therapy educational program per CAPTE.
• Current license to practice physical therapy in the assigned state.
• Driver's License and access to reliable transportation.
• Current CPR certification.

Responsibilities

• Help patients maintain, restore and improve movement, activity and function.
• Provide interventions, conduct reexaminations every 30 days, modify interventions, and perform discharge planning.
• Other duties as assigned.
