Job summary

Amedisys, part of the Optum family, is seeking a Physical Therapist - HAC to restore function and improve mobility in patients at home. You will provide skilled PT services under a physician-directed plan of care within an interdisciplinary model.

Responsibilities include assessments, treatment planning, and coordination with physicians, with ongoing documentation and care planning support. Licensing, CAPTE accreditation, CPR, and driver’s license are required.

Qualifications

• Current, unencumbered license to practice physical therapy in the assigned state.
• Graduate of a CAPTE-accredited physical therapy program.
• 1+ year of experience in physical therapy; OR less than one year with residency/mentorship.
• Current CPR certification.
• 1+ year of experience in clinical assessment, documentation, and care coordination.
• Driver's License and access to reliable transportation.

Responsibilities

• Conduct comprehensive patient assessments and develop individualized PT plans of care in collaboration with physicians.
• Provide skilled PT interventions and modify treatment plans based on patient progress.
• Perform ongoing reassessments, including weekly evaluations, to support care planning and physician coordination.
• Collaborate with interdisciplinary teams to coordinate care and communicate patient progress and condition changes.
• Maintain accurate, timely documentation in compliance with standards.
