Job summary

Amedisys, part of the Optum family, is seeking a Physical Therapist (HAC) in a home health setting. You will assess patients, plan and provide therapy, and coordinate care with physicians and teams.

The role requires state licensure, CAPTE accreditation, CPR, and 1+ year of PT experience with documentation and coordination. Salary ranges from $86,184 to $105,336 annually for full-time employment.

Qualifications

• Current, unencumbered license to practice physical therapy in the assigned state.
• Graduate of an accredited physical therapy program (CAPTE accredited).
• 1+ year of experience in physical therapy; OR less than one year with residency or mentorship.
• Current CPR certification.
• 1+ year of experience in clinical assessment, documentation, and care coordination.
• Driver's License and access to reliable transportation.

Responsibilities

• Conduct comprehensive patient assessments and develop individualized physical therapy plans of care in collaboration with physicians.
• Provide skilled physical therapy interventions and modify treatment plans based on patient progress and clinical needs.
• Perform ongoing reassessments, including weekly evaluations, to support care planning and physician coordination.
• Collaborate with interdisciplinary teams to coordinate care and communicate patient progress and condition changes.
• Maintain accurate, timely documentation in compliance with clinical, regulatory, and documentation standards.
