Amedisys, part of the Optum family, is seeking a Physical Therapist – HAC to help patients restore function and mobility through physician-guided plans of care in a home health setting.

You will perform assessments, develop plans, coordinate care with interdisciplinary teams, document accurately, and contribute to a compassionate workplace that supports growth and recognition.

Formación

• Graduate of CAPTE accredited physical therapy program.
• Current, unencumbered physical therapy license in the assigned state.
• Current CPR certification.
• 1+ year of physical therapy experience; or less with residency/mentorship.
• 1+ year experience in clinical assessment, documentation, and care coordination.
• Driver's license and reliable transportation.

Responsabilidades

• Conduct comprehensive patient assessments and develop individualized physical therapy plans of care with physicians.
• Provide skilled physical therapy interventions and modify plans based on progress and needs.
• Perform ongoing reassessments, including weekly evaluations, to support care planning.
• Coordinate care with interdisciplinary teams and communicate progress and changes.
• Maintain accurate documentation in compliance with standards.
