Job summary

The Pennant Group, Inc. is seeking a home health physical therapist contracted or employed through Nurses on Wheels to provide PT services in patients' homes and ensure adherence to the personnel services agreement.

This role includes evaluating patients, developing rehabilitation plans, and coordinating discharge planning with nursing teams. Qualified candidates hold a state-licensed PT with a baccalaureate or master’s degree, have 2+ years of PT experience (home health preferred), strong

Qualifications

• Degree in physical therapy from a baccalaureate or master’s program.
• Licensed to practice as a physical therapist within the state.
• Two years of experience as a physical therapist; home health experience preferred.
• Good verbal and written communication and organization skills.
• CPR certification current.
• Licensed driver with automobile in good condition and insured.

Responsibilities

• Provide physical therapy services according to physician’s plan of care and state practice act.
• Assess and evaluate functional status and contribute to the plan of care.
• Evaluate home environment for safety and rehabilitation potential.
• Direct physical therapy treatment and supervise assistants as allowed by policy.
• Instruct patients and families in use and care of therapeutic devices and goals of therapy.
• Document progress and discharge planning per organization policy.
