Job summary

The Pennant Group is seeking a licensed physical therapist to provide comprehensive home health care in Oregon. You will collaboratively develop and implement individualized care plans with patients and caregivers, while ensuring adherence to physician orders and organizational policies.

Responsibilities include counseling, patient education, documentation, and coordination with medical staff. Prior experience in community/home health is preferred; driving is required.

Qualifications

• Licensed to practice as a physical therapist in the state
• Two years of experience as a physical therapist
• CPR certification maintained
• Valid driver’s license and reliable transportation

Responsibilities

• Provide physical therapy services per physician’s plan of care
• Develop and update individualized care plans with patient and family
• Counsel patient, caregiver, and family; educate on therapies and devices
• Communicate with physicians and other clinicians about progression
• Supervise PT Assistants as required by policy and state regulations
• Participate in discharge planning and quality improvement programs
