Job summary

Nurses on Wheels in Texas is seeking a licensed physical therapist to provide home health services contracted or employed by the agency. You will follow physician plans of care and state PT guidelines to assess, treat, and educate patients in their homes.

The role requires a PT degree, current state licensure, CPR certification, and a valid driver's license with a reliable vehicle. Prior home health or community-based experience is preferred.

Qualifications

• Bachelor's or Master’s degree in Physical Therapy approved by an accredited organization.
• Licensed to practice as a Physical Therapist in the state.
• Two years of physical therapy experience; community/home health preferred.
• Excellent verbal and written communication and organizational skills.
• Current CPR certification.
• Licensed driver with a reliable vehicle.

Responsibilities

• Provides physical therapy services to patients per physician's plan of care and state Physical Therapy Practice Act.
• Assessing and evaluating therapeutic/rehabilitative/functional status; participates in the total plan of care; evaluates home environment.
• Assesses muscle strength, mobility, gait, ROM; evaluates rehab potential.
• Directs physical therapy treatment.
• Instructs patients and families/caregivers in use and care of therapeutic appliances.
• Determines priority needs for physical therapy.
• Reports patient’s reaction to treatment or changes in condition to physician.
• Initiates PT program and trains other personnel/family in therapy goals.
• Identifies needs for other home health services and refers as necessary.
• Prepares and submits clinical/progress summaries.
• Provides PT consultation to home families/caregivers.
• Supervises PT assistants per policy and state regulations.
• Instructs and evaluates home health aides every two weeks when therapy is the only skilled service.
