Qualifications

• Licensed to practice as physical therapist in the state.
• CAPTE-accredited degree in physical therapy.
• Two years of relevant PT experience.
• Home health experience preferred.
• Current CPR certification.

Responsibilities

• Develops and updates an individualized plan of care with patient, family, and caregivers.
• Provides PT services per physician plan of care and educates patient and family.
• Assesses functional status, gait, mobility, and ROM; directs therapy sessions.
• Prepares clinical notes and progress summaries; communicates with physician and care team.
• Participates in discharge planning and coordinates with home health services.
