Job summary

The Pennant Group, Inc. is seeking a licensed physical therapist to provide services within home health and related settings across the Abilene area.

The role involves collaborating with patients, families, and care teams to implement and adjust individualized plans of care. Responsibilities include assessment, treatment, patient and caregiver education, documentation, and communication with the physician and other health professionals.

Qualifications

• Licensed to practice as physical therapist within the state.
• CAPTE-accredited degree (baccalaureate or master’s) in physical therapy.
• Two years of experience as a physical therapist; home health preferred.
• Must have CPR certification and valid driver’s license with access to a vehicle.

Responsibilities

• Assesses and develops the plan of care with patient, representative, and caregiver(s).
• Provides patient, caregiver, and family counseling and education.
• Documents clinical notes and progress summaries tied to goals.
• Communicates with physician and other health care professionals regarding plan of care.
• Delivers PT services per physician’s plan; evaluates status, mobility, ROM, and rehab potential.
• Trains patient and family in device use; participates in discharge planning.
