The Pennant Group, Inc. is seeking a licensed physical therapist to provide home health PT services under an individualized plan of care. The role emphasizes assessment, patient education, and coordination with physicians and caregivers to optimize functional outcomes.

The successful candidate will hold a CAPTE-approved PT degree, maintain CPR certification, and have at least two years of PT experience, including home health. Travel to patient homes is required.

Competenze

• Licensed to practice as a physical therapist in the state
• Degree from a CAPTE-accredited program (baccalaureate or master's)
• Two years of PT experience; home health preferred
• Good verbal and written communication skills
• Current CPR certification and ability to travel to patients' homes

Mansioni

• Conduct interdisciplinary assessment and develop the individualized plan of care with patient and caregiver(s)
• Provide patient, caregiver, and family counseling and education
• Document clinical notes and progress summaries
• Communicate with physician and other health professionals about the plan of care
• Deliver PT services per physician plan of care, assess status and rehab potential
• Instruct patient and family on use of therapeutic devices and home exercises
• Identify needs for additional home health services and coordinate referrals
• Participate in discharge planning and quality improvement activities
