Job summary

Nurses On Wheels is seeking a licensed Physical Therapist to provide home health services contracted or employed. The PT will implement the physician's plan of care, assess functional status, and coordinate with families and caregivers in the home setting.

The role requires state licensure, CPR, and a valid driver's license, with 2 years of physical therapy experience. Community/home health experience is preferred, and the provider will work closely with the care team to achieve patient goals.

Qualifications

• Degree from a baccalaureate or Master’s program in physical therapy approved by an accredited organization.
• Licensed to practice as a physical therapist in the state, if applicable.
• Two years of appropriate experience as a physical therapist; community/home health preferred.
• Demonstrates good verbal and written communication and organizational skills.
• Possesses and maintains current CPR certification.
• Licensed driver with automobile in good working order and insured as required.

Responsibilities

• Provides physical therapy services to patients according to a physician's plan of care and state laws.
• Assesses patient status and develops the total plan of care; evaluates home environment.
• Directs physical therapy treatment and educates patients and families on therapy use.
• Identifies patient and family needs for other home health services and refers as necessary.
• Prepares and submits progress summaries and discharge planning documents as directed.
