Job summary

BrightStar Care – Concord, Lexington & Woburn is seeking a licensed Physical Therapist to join our in-home care team. This part-time/per diem role focuses on delivering skilled PT services directly in patients' homes, improving strength, balance, mobility, and safety.

Perform in-home evaluations, develop personalized care plans, educate clients and families, and collaborate with a multidisciplinary team, using a mobile EMR for documentation. A MA license and CPR certification are required.

Qualifications

• Active MA PT license is required.
• CPR certification is required.
• DPT or MPT from an accredited program is required.
• 1+ year of experience preferred (new grads welcome).
• Reliable vehicle and valid driver's license.

Responsibilities

• Perform in-home PT evaluations and create personalized care plans.
• Provide skilled PT interventions to enhance strength, balance, mobility, and safety.
• Educate clients and families on exercises and home safety.
• Collaborate with a multidisciplinary care team.
• Document care using mobile-friendly EMR.
