Job summary

BrightStar Care – Concord, Lexington & Woburn is seeking a licensed Physical Therapist to perform in-home evaluations and provide skilled PT interventions that restore strength, balance, and independence for homebound clients. This part-time/per diem role offers flexible scheduling and mileage reimbursement.

You will join a supportive team with a mobile EMR, clinical leadership, and opportunities for ongoing training and career growth within a trusted home health provider.

Qualifications

• Active MA PT license
• DPT or MPT from an accredited program
• CPR certification
• 1+ year experience preferred (new grads welcome)
• Reliable vehicle and valid driver’s license

Responsibilities

• Perform in-home PT evaluations and develop personalized care plans
• Provide skilled PT interventions to improve strength, balance, mobility, and safety
• Educate clients and families on exercises and home safety
• Collaborate with multidisciplinary care team
• Document care using mobile-friendly EMR
