Job summary

BrightStar Care – Concord, Lexington & Woburn is seeking a licensed Physical Therapist to provide in-home PT services on a part-time or per diem basis. You will assess clients, create personalized treatment plans, and help restore strength, balance, and independence in familiar home environments.

Ideal candidates hold MA licensure (DPT or MPT preferred), CPR certification, and at least 1 year of experience, with new grads welcome. A reliable vehicle and valid driver’s license are required.

Qualifications

• Active MA PT license.
• DPT or MPT from an accredited program.
• CPR certified.
• 1+ year of experience preferred (new grads welcome)
• Reliable vehicle & valid driver’s license.

Responsibilities

• Perform in-home PT evaluations and create personalized care plans.
• Provide skilled PT interventions to enhance strength, balance, mobility, and safety.
• Educate clients and families on exercises and home safety.
• Collaborate with a multidisciplinary care team.
• Document care using mobile-friendly EMR.
