Job summary

BrightStar Care of Concord, Lexington and Woburn is seeking a licensed Physical Therapist to provide in-home and facility-based services with a flexible schedule. You will evaluate, treat, and educate patients to improve strength, balance and mobility in their own homes.

Work includes documenting in a mobile EMR, collaborating with nurses, OTs, HHAs and case managers, and delivering high-quality care aligned with BrightStar’s standards. Local visits; growth and CE support offered.

Qualifications

• Active and unrestricted Massachusetts Physical Therapist license.
• Graduate of an accredited PT program (DPT or MPT).
• Minimum 1 year PT experience preferred (new grads welcome with supervision).
• Reliable vehicle and valid driver’s license.
• CPR certification.
• Ability to work independently and as part of a care team.

Responsibilities

• Perform in-home PT evaluations and develop personalized care plans.
• Deliver skilled therapy focused on strength, balance, gait, and mobility.
• Educate patients and families on home exercise programs and safe mobility.
• Collaborate with nurses, OT, HHAs, and case managers.
• Document progress and visit notes using a mobile EMR.
• Ensure patient safety and quality outcomes per BrightStar standards.
