Job summary

BrightStar Care of Concord, Lexington, and Woburn is seeking a licensed Physical Therapist to join our home-health team on a part-time/per diem basis. You will perform in-home PT evaluations, create personalized care plans, and deliver interventions to improve strength, balance, mobility, and safety for clients in the settings they cherish.

Active MA PT license and CPR are required; DPT or MPT from an accredited program. New grads welcome.

Qualifications

• Active MA PT license
• CPR certified
• DPT or MPT from an accredited program

Responsibilities

• Perform in-home PT evaluations and create personalized care plans
• Provide skilled PT interventions to enhance strength, balance, mobility, and safety
• Educate clients and families on exercises and home safety
• Collaborate with a multidisciplinary care team
• Document care using mobile-friendly EMR
