Job summary

BrightStar Care of Concord, Lexington, and Woburn is seeking a licensed Physical Therapist (PT) for in-home care on a part-time / per diem basis. You will evaluate clients in their homes and develop personalized care plans to restore strength, balance, mobility, and independence.

Qualifications

• Active MA PT license.
• DPT or MPT from an accredited program.
• CPR certified.
• 1+ year of experience preferred (new grads welcome).
• Reliable vehicle & valid driver’s license.

Responsibilities

• Perform in-home PT evaluations and create personalized care plans.
• Provide skilled PT interventions to enhance strength, balance, mobility, and safety.
• Educate clients and families on exercises and home safety.
• Collaborate with a multidisciplinary care team.
• Document care using mobile-friendly EMR.
