Job summary

BrightStar Care is seeking a licensed Physical Therapist (PT) to join our Concord, Lexington & Woburn team for part-time or per diem in-home care. You will help clients regain strength, balance, mobility, and independence in the familiar surroundings of their homes.

Responsibilities include in-home evaluations, personalized care plans, educating clients and families, documenting care using mobile EMR, and collaborating with a multidisciplinary team.

Qualifications

• Active MA PT license
• DPT or MPT from an accredited program
• CPR certified
• 1+ year of experience preferred (new grads welcome)
• Reliable vehicle & valid driver’s license

Responsibilities

• Perform in-home PT evaluations and create personalized care plans
• Provide skilled PT interventions to enhance strength, balance, mobility, and safety
• Educate clients and families on exercises and home safety
• Collaborate with a multidisciplinary care team
• Document care using mobile-friendly EMR
