Powerback Rehabilitation To You is seeking a Physical Therapist to deliver in-home outpatient rehabilitation for older adults. You will assess needs, create personalized therapy plans, and work with caregivers to promote independence and safe mobility in the home.

The role emphasizes flexible scheduling, professional collaboration, and ongoing education through a mentorship program and CEUs. A strong focus on compassionate care and clinical excellence is expected.

Qualifications

• Valid Physical Therapy license in the state(s) of practice.
• Master's degree in Physical Therapy; or PT-related field with BS in PT.
• Bachelor's degree in Physical Therapy + 5 years of PT experience.

Responsibilities

• Provide outpatient rehabilitation services in-home for older adults.
• Evaluate and treat in the patient's home or living facility.
• Develop and deliver individualized PT programs to restore function.
• Instruct patients and caregivers in skills to promote independence.
• Supervise PT assistants and other staff as allowed by licensure.
• Document clinical progress and maintain accurate records.
