Job summary

PowerBack Rehabilitation is seeking a Physical Therapist for full-time mobile outpatient therapy across Westfield, Monson, and Holyoke. The role delivers in-home outpatient rehabilitation for older adults, with a flexible schedule and a posted salary range of $80,000 to $110,000 per year plus a $5,000 bonus.

Responsibilities include evaluating and treating patients in their homes or living facilities, developing individualized plans, and training caregivers.

Qualifications

• Must have a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility.
• Master’s degree in Physical Therapy; or Master’s degree in a PT-related field in addition to a Bachelor’s degree in Physical Therapy; or Bachelor’s degree in Physical Therapy plus 5 years of progressively responsible PT experience.
• Must have a valid driver’s license or reliable, independent transportation to and from the patients’ residence.

Responsibilities

• Provide outpatient rehabilitation services in-home for the older adult population.
• Evaluate and treat in the patient’s home or in an independent living facility.
• Develop and deliver individualized physical therapy plans.
• Educate patients and caregivers to promote independence.
• Supervise PT Assistants when applicable.
• Maintain documentation and adhere to scheduling and billing requirements.
