Powerback Rehabilitation seeks a Physical Therapist to serve Hampden County in an in-home outpatient setting, delivering rehabilitation services to older adults in private residences or independent living facilities. The PT evaluates needs and designs individualized treatment plans to maximize function.

The role includes collaborating with the rehabilitation team, supervising staff as needed, and educating patients and caregivers to promote independence.

Qualificações

• Must have a valid Physical Therapy license in the state(s) of practice.
• Master’s degree in Physical Therapy; or an acceptable combination as outlined above.
• Must have a driver’s license or reliable transportation to patients’ residences.

Responsabilidades

• Provide outpatient rehabilitation services in in-home or independent living settings.
• Assess patients, develop and implement therapy plans to restore function.
• Supervise PT Assistants and other staff as required.
• Maintain documentation and scheduling in a timely manner.
• Educate patients and caregivers to promote independence.
