Job summary

Powerback Rehabilitation to You is seeking a Physical Therapist to deliver outpatient rehabilitation services in home settings for older adults. You will evaluate needs, design individualized plans, and coordinate with the rehab team to support recovery and independence.

Qualifications

• Must hold a valid Physical Therapy license in the state of practice.
• Master's degree in Physical Therapy or Master’s in a related field with a Bachelor's in Physical Therapy.
• Must have a valid driver’s license or reliable transportation to patients' residences.

Responsibilities

• Provide outpatient rehabilitation services in in-home settings for older adults.
• Assess needs and develop individualized physical therapy plans and refer to other disciplines as needed.
• Deliver education to patients and caregivers to promote independence and safe home mobility.
• Coordinate with the rehabilitation team to select appropriate therapy regimens and adapt plans as needed.
• Ensure documentation, scheduling, and safety compliance in all patient interactions.
