Job summary

Powerback Rehabilitation To You is seeking a Physical Therapist to provide in-home outpatient therapy for older adults across various Massachusetts communities. This full-time role offers a $5,000 sign-on bonus and a flexible schedule within patient homes and community settings.

Responsibilities include evaluating patients, creating customized therapy plans, teaching exercises to patients and caregivers, and coordinating with the rehabilitation team to restore function and quality of life.

Qualifications

• Must have a valid Physical Therapy license in state(s) of practice or license eligibility.
• Masters degree in Physical Therapy or a related field plus Bachelor's in Physical Therapy and experience may be acceptable.

Responsibilities

• Provide in-home outpatient PT for older adults.
• Develop and adjust individualized therapy programs.
• Educate patients and caregivers on exercises and precautions.
• Collaborate with rehabilitation team to coordinate care.
• Maintain safety and document patient progress and outcomes.
