You will assess, plan, and implement therapy programs in patients' homes or in living facilities, coordinating with the care team and families to promote independence.

Qualifications

• Must have a valid Physical Therapy license in the state(s) of practice.
• Master's degree in Physical Therapy or equivalent.
• Bachelor's degree in Physical Therapy with 5 years of progressive PT experience.

Responsibilities

• Provide outpatient rehabilitation services in home or facility settings to older adults.
• Develop and implement individualized therapy programs to restore and enhance function.
• Collaborate with rehabilitation team and refer to other disciplines as needed.
• Educate patients and caregivers on skills and home exercise programs.
• Supervise assistants and ensure timely documentation and scheduling.
