Job summary

Powerback Rehabilitation to You seeks a Physical Therapist for mobile outpatient services in Watertown, CT. The position is PRN and focuses on home-based and community settings, delivering patient-centered rehabilitation.

The PT assesses needs, designs individualized therapy programs, coordinates with caregivers, and promotes independence with functional activities. A state PT license is required along with reliable transportation and a commitment to high-quality care.

Qualifications

• Must have a valid Physical Therapy license in the state(s) of practice.
• Master's degree in Physical Therapy or an equivalent with licensure eligibility.
• Bachelor's degree in Physical Therapy plus related experience acceptable.
• Valid driver’s license or reliable transportation to patients’ homes.

Responsibilities

• Supervise PTAs, temporary licensees, students and support personnel as allowed by state rules.
• Adhere to department schedules, documentation, and attend care meetings.
• Develop and adapt therapy plans to restore function and promote independence.
• Provide home-based outpatient therapy and in-facility care as needed.
• Educate patients and caregivers on exercises and safety to prevent falls.
