Job summary

PowerBack Rehabilitation is seeking a Physical Therapist to provide in-home outpatient rehabilitation services for older adults in Massachusetts. This PRN role emphasizes personalized, patient-centered care in home settings, enabling independence and functional improvement.

You will assess needs, design and implement PT programs, educate patients and caregivers, and coordinate with a multidisciplinary team to optimize outcomes in home and community environments.

Qualifications

• Must have a valid Physical Therapy license in the state(s) of practice.
• Master's degree in Physical Therapy.
• Driver's license or reliable transportation to and from patients' homes.

Responsibilities

• Supervise Physical Therapy Assistants, temporary licensees, students and support personnel as required.
• Adhere to department and patient treatment schedules; complete clinical and billing documentation.
• Attend patient care and staffing conferences and related meetings.
• Refer to other disciplines and agencies to address patient needs.
• Collaborate with rehab team to select appropriate therapeutic regimens.
• Adapt programs and plans of care to individual patient needs.
• Promote a safe environment and remove hazards.
• Order supplies and equipment as necessary.
• Maintain and report on equipment condition and repairs.
• Deliver patient-centered care and educate patients and caregivers.
