Powerback Rehabilitation To You is seeking a PRN Physical Therapist to deliver outpatient rehabilitation in in-home and facility settings for older adults across NH. You will evaluate needs, develop and implement physical therapy plans, and collaborate with a multidisciplinary team to restore function and independence.

Qualifications

• Must have a valid Physical Therapy license in the state(s) of practice.
• Master's degree in Physical Therapy or related field with PT degree.
• Foreign PT degree evaluated for U.S. equivalency if earned abroad.
• Optional: Driver's license or reliable transportation.

Responsibilities

• Supervise PT Assistants, temporary licensees, students and support personnel as needed and per state licensure.
• Adhere to department and patient treatment schedules; complete clinical and billing documentation.
• Attend patient care meetings and contribute to staffing conferences.
• Refer patients to other disciplines to address comprehensive needs.
• Collaborate with rehab team to select appropriate therapy regimen.
• Adapt plan of care to individual patient needs and progress.
• Promote a safe environment and ensure safety hazards are eliminated.
• Order supplies and equipment as necessary.
• Maintain and report on equipment condition and repairs.
• Put patient service first and ensure high-quality care and compassionate service.
• Evaluate patients: history, systems review, tests, determine diagnosis, plan of care, and progress; cover various therapy techniques.
• Develop and apply adaptive devices as needed; training in use of orthotics/prosthetics.
• Select activities aligned with patient capacity and interests.
• Instruct mobility skills and assess progress; modify plans as needed.
• Perform routine reexaminations and integrate wellness programs.
• Conduct home/community mobility assessments and provide safe mobility recommendations.
• Perform other related duties as required.
