Qualifications

• Must hold a current Physical Therapy license in the state of practice.
• Master's degree in Physical Therapy or equivalent.
• Bachelor's degree in Physical Therapy plus experience may meet criteria.

Responsibilities

• Provide outpatient rehabilitation in home or community settings.
• Develop and implement individualized therapy plans.
• Assess, treat, and document patient progress and outcomes.
• Educate patients and caregivers on home exercise and safety."
• Coordinate with rehabilitation team to optimize care.
