Job summary

Powerback Rehabilitation, LLC seeks a Physical Therapist for in-home outpatient services in New Jersey. The role involves evaluating and treating older adult patients in their homes or in independent living facilities, with a flexible, patient-centered approach.

You will work within a comprehensive rehab team, design individualized therapy programs, and educate patients and caregivers to promote independence and mobility.

Qualifications

• Must hold a valid Physical Therapy license.
• Master's degree in Physical Therapy or equivalent.
• Bachelor's degree in Physical Therapy with 5 years of experience.
• Foreign PT degree must be evaluated for US equivalence.
• Must have a valid driver's license or reliable transport.

Responsibilities

• Supervise Physical Therapy Assistants, temporary licensees, students and support staff per state licensure.
• Adhere to department and patient treatment schedules; complete documentation and attend meetings.
• Attend patient care and staffing conferences; contribute where needed.
• Refer to other disciplines to address patient needs; collaborate with rehab team.
• Adapt plan of care based on patient needs; promote safety and report hazards.
• Order supplies and equipment; maintain and repair equipment as needed.
• Deliver high-quality patient service in a caring environment and respect patient rights.
• Conduct comprehensive patient assessments and design, implement, and adjust therapy plans.
• Provide home and community mobility assessments and recommendations.
