Job summary

Powerback Rehabilitation to You is seeking a Physical Therapist to provide in-home outpatient services for older adults in the West Hartford area. This PRN role offers a flexible 10 hours per week schedule and a mission to help patients regain strength and independence in familiar surroundings.

The therapist will assess needs, develop individualized plans, educate patients and caregivers, and maintain accurate documentation while collaborating with the care team to optimize outcomes.

Qualifications

• Must have a valid Physical Therapy license.
• Master’s degree in Physical Therapy or PT-related Master’s with Bachelor’s.
• Foreign PT degree must be evaluated to the U.S. equivalent.
• Valid driver’s license or reliable transportation to and from patients’ residences.

Responsibilities

• Provide outpatient rehabilitation services in-home for older adults.
• Deliver individualized PT programs to restore and enhance function.
• Educate patients and caregivers in home exercise and safety.
• Coordinate with rehabilitation team for comprehensive care.
• Maintain timely documentation and patient progress notes.
