Job summary

Powerback Rehabilitation To You is seeking a Physical Therapist to provide in-home outpatient rehabilitation to older adults. You will evaluate needs, develop personalized therapy plans, and educate patients and caregivers on home exercises to promote independence.

Responsibilities include coordinating with the rehab team, maintaining documentation, and contributing to a flexible in-home care model across various settings. PRN approx 10 hours per week, with opportunities for growth.

Qualifications

• Must have a valid Physical Therapy license in the state of practice.
• Master's degree in Physical Therapy.
• Driver's license or reliable transportation.
• Experience in home-based outpatient therapy preferred.

Responsibilities

• Provide in-home outpatient PT services to the older adult population.
• Develop and implement individualized therapy programs.
• Educate patients and caregivers on home exercises and safety.
• Coordinate with rehabilitation team to address comprehensive needs.
• Ensure timely documentation and adherence to schedule.
