Job summary

BrightStar Care – Concord, Lexington & Woburn is seeking a licensed Physical Therapist (PT) to perform in-home evaluations and deliver personalized care plans. This part-time/per diem role offers flexible scheduling and mileage reimbursement as you support clients in their home environments.

Requirements include an active MA PT license, a DPT or MPT from an accredited program, CPR certification, and reliable vehicle. New grads welcome to apply for local home visits.

Qualifications

• Active MA PT license.
• DPT or MPT from an accredited program.
• CPR certified.
• 1+ year of in-home PT experience preferred (new grads welcome).
• Reliable vehicle and valid driver’s license.

Responsibilities

• Perform in-home PT evaluations and create personalized care plans.
• Provide skilled PT interventions to improve strength, balance, mobility, and safety.
• Educate clients and families on exercises and home safety.
• Collaborate with a multidisciplinary care team.
• Document care using mobile EMR.
