Job summary

Powerback is seeking a Physical Therapist to deliver outpatient rehabilitation services in patients’ homes or in independent living facilities across Bedford, NH and surrounding areas. The role emphasizes functional, home-based therapy with a focus on restoring independence and mobility.

The candidate must hold a valid PT license in the state, possess a Master’s degree in Physical Therapy (or equivalent), and have reliable transportation for home visits.

Qualifications

• Must have a valid Physical Therapy license in the state of practice.
• Master’s degree in Physical Therapy (or PT-related field with Bachelor’s in PT).
• Must have a valid driver’s license or reliable independent transportation to and from patients’ residence.

Responsibilities

• Provide outpatient rehabilitation services in-home for older adults in private homes or independent living facilities.
• Evaluate and treat in the most functional environment; develop and deliver PT programs; refer to other disciplines as needed.
