Job summary

PowerBack Rehabilitation is seeking a Physical Therapist to provide in-home outpatient therapy for the elderly, delivering evaluation and treatment in patients’ homes or community settings across NH. You will design individualized therapy plans, coordinate with the rehab team, and educate patients and caregivers to promote independence.

The role requires a valid PT license in the applicable state, and a Master's degree in Physical Therapy or equivalent.

Qualifications

• Must have a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility.
• Must have a Master's degree in Physical Therapy; or a Master's in PT plus a Bachelor's in PT; or a Bachelor's in PT with 5 years of PT experience.

Responsibilities

• Provide outpatient rehabilitation services in in-home settings for older adults.
• Evaluate and treat in the most functional environment, the patient's home.
• Develop and deliver individualized therapy programs to restore, reinforce, and enhance performance.
• Instruct and train patients and caregivers in essential skills for independence.
• Supervise PT assistants, temporary licensees, students and support personnel where applicable.
• Manage documentation, scheduling, and coordination with other disciplines.
