Job summary

21st Century Home Health Services (21HHS) is hiring a Full Time Physical Therapist Case Manager to serve Placer County. You will evaluate patients in their homes, create care plans, and work with PTAs to implement therapy aimed at restoring function and independence.

Applicants must hold an unrestricted CA PT license, CPR/BLS, and a CA driver’s license. Experience in home health or case management is preferred, with autonomous field-based work emphasized.

Qualifications

• CA Physical Therapist license (unrestricted) required.
• Current BLS/CPR certification.
• Valid CA driver’s license.
• Experience in home health or case management preferred.
• Ability to work independently in patient homes.

Responsibilities

• Perform in-home evaluations and develop care plans with PTAs to follow.
• Restore function and improve independence through targeted PT interventions.
• Coordinate with physicians to ensure high-quality care and timely progress reports.
• Communicate progress and adjust treatment plans as needed.
