Powerback Rehabilitation To You seeks a Physical Therapist Assistant to deliver outpatient rehabilitation in patients' homes across NJ. You will evaluate, treat, and design PT programs for older adults in private residences or community living settings.

The role emphasizes caregiver education and collaborative care to foster independence. The ideal candidate holds a PT license (state-appropriate) and is ready for a flexible schedule with travel across designated territories.

Qualifications

• Must have a valid Physical Therapy license in the state(s) of practice.
• Master's degree in Physical Therapy; or
• Master's degree in Physical Therapy related field plus Bachelor's in Physical Therapy; or
• Bachelor's degree in Physical Therapy with 5 years of progressive PT experience.

Responsibilities

• Provide outpatient rehabilitation services in the home environment for older adults.
• Evaluate, develop, and deliver physical therapy programs to facilitate rehabilitation.
• Educate patients and caregivers to promote independence and safe home mobility.
• Coordinate referrals to other disciplines to address comprehensive needs.
