Job summary

Thom Child & Family Services in Norwood, MA is seeking a licensed Physical Therapist to join our Early Intervention team. The clinician will provide home- and community-based services to infants and toddlers with developmental delays or at risk, working closely with families to tailor plans and ensure family-centered, evidence-based practice.

You will coordinate services, participate in IFSP development, and support families through coaching, assessments, and ongoing documentation.

Qualifications

• Current licensure as a Physical Therapist by the Massachusetts Board of Registration of Allied Health Professions.
• Experience with infants, toddlers and families is preferred.
• Knowledge of family systems and early childhood development.

Responsibilities

• Participate in multidisciplinary team assessments of the client’s developmental status.
• Assess family and child strengths/resources, concerns, and progress.
• Provide child and family centered services and activities developmentally appropriate for children birth to three and their families using parent coaching model.
• Provide service coordination by scheduling, developing, implementing, monitoring, and evaluating the intervention plan for each family, ensuring that appropriate procedures and timelines are followed.
• Partner with families in the development of Individual Family Service Plans (IFSPs) to identify goals, objectives, and strategies related to the child’s development.
• Facilitate parent-child interactions to encourage child's development; assist families to act as advocates for their child.
• Complete progress notes, reports, assessments, and correspondence as appropriate using the Thom electronic documentation system.
• Provide individual and group intervention for infants and toddlers from birth to three years old who have developmental delays or who are at risk for delays in the areas of cognition, adaptive behavior, social-emotional development, self-care, fine motor, gross motor, and communication.
