Job summary

Chapters Health System is seeking an RN Case Manager for the Float Team to deliver high-quality hospice and palliative care in patients’ homes across assigned Florida locations, including Wesley Chapel. You will assess patient needs, develop and coordinate the Plan of Care with the Interdisciplinary Team (IDT), and ensure compassionate support to patients and families.

The role requires an active Florida RN license and at least 1 year of nursing experience (hospice or hospital preferred),

Qualifications

• Current license as RN in the state where the employee will be working.
• Minimum of one (1) year nursing experience; hospice or hospital experience preferred.
• Experience with EMR/EHR systems.
• Mobile driver with valid license and automobile insurance.
• Reliable transportation to meet visit schedule.
• Ability to communicate effectively in English (verbal and written).
• Ability to visit participants in their homes for assessments.
• Able to perform essential functions including lifting patients/equipment.
• Active BLS for healthcare professionals from AHA/Red Cross.

Responsibilities

• Provides and manages direct care to patients and families as part of the IDT with appropriate nursing interventions.
• Coordinates the Plan of Care reflecting patient needs.
• Educates patients and families about disease, care processes and safety practices.
• Reports changes to IDT and collaborates on referrals and admissions.
• Communicates clearly with physicians, staff, patients and families; SBAR documentation.
• Supervises home health aides as requested.
• Participates in emergencies and safety protocols as needed.
