Job summary

Beyond Home Health Care is seeking a licensed Occupational Therapist to provide in-home and community-based OT services to adults with developmental disabilities in New Jersey. You will evaluate functional performance, create person-centered treatment plans, and deliver interventions to maximize independence in ADLs and IADLs.

• Current New Jersey OT license issued by the NJ State Board of Occupational Therapy.
• ACOTE-accredited OT program completion.
• NBCOT certification preferred; required for new graduates.

Responsibilities

• Conduct comprehensive evaluations and develop individualized treatment plans aligned with ISP and person-centered goals.
• Deliver one-on-one OT services in home and community settings.
• Document services in accordance with DDD standards and required timelines.
• Collaborate with Support Coordinators, families, and interdisciplinary team to maintain continuity of care.
• Provide caregiver training and home programs to reinforce goals.
