Job summary

Beyond Home Health Care in New Jersey seeks a licensed Occupational Therapist to provide in-home and community-based OT services to adults with developmental disabilities. The OT will evaluate functional performance, create person-centered treatment plans, and deliver interventions to maximize independence in ADLs, IADLs, sensory processing, and fine motor skills.

Key duties include ISP-aligned evaluations, home/community-based therapy, thorough documentation, and collaboration with supports

Qualifications

• Current New Jersey license as an Occupational Therapist (OT).
• Graduate of an ACOTE-accredited OT program.
• NBCOT certification preferred; required for new graduates.
• Minimum of one year of clinical experience preferred; relevant fieldwork considered.
• CPR/BLS certification.
• Clean background check and fingerprinting per NJ DDD requirements.
• Reliable transportation and valid NJ driver’s license.
• Ability to pass DDD background checks (CRoff).

Responsibilities

• Conduct comprehensive evaluations and develop individualized treatment plans aligned with ISP and person-centered goals.
• Deliver one-on-one OT services in homes and community settings.
• Document services per DDD standards (notes, progress, outcomes) within required timeframes.
• Collaborate with Support Coordinators, families, caregivers, and the interdisciplinary team.
• Provide caregiver training and home programs to reinforce goals.
• Adhere to DDD policies and HIPAA confidentiality standards.
• Maintain accurate billing records consistent with DDD rates and authorizations.
