Zusammenfassung

Beyond Home Health Care is seeking a licensed Occupational Therapist to deliver in-home and community-based OT services to adults with developmental disabilities across New Jersey. You will evaluate function, develop person-centered plans, and improve ADLs, IADLs, and community participation according to NJ DDD guidelines.

Responsibilities include home-based therapy, progress documentation, caregiver coaching, and coordination with supports coordinators.

Qualifikationen

New Jersey license as an Occupational Therapist issued by the NJ State Board.
Graduate of an ACOTE‑accredited OT program.
NBCOT certification preferred; required for new graduates.
Minimum of 1 year of clinical experience preferred; fieldwork acceptable.
Current CPR/BLS certification and clean background check per NJ DDD.

Aufgaben

Conduct comprehensive in-home evaluations and develop ISP-aligned treatment plans.
Deliver one-on-one OT services in participants' homes and community settings.
Document sessions, progress notes, and outcomes per DDD standards.
Collaborate with coordinators, families, and the interdisciplinary team.
Provide caregiver training and home programs to reinforce goals.
Adhere to DDD policies and HIPAA confidentiality requirements.
Maintain billing records consistent with DDD rate structures.
