Job summary

Beyond Home Health Care is seeking a licensed Occupational Therapist to provide in-home and community-based OT services to adults with developmental disabilities in New Jersey. You will evaluate functional performance, create person-centered plans, and deliver interventions to maximize ADLs, IADLs, and community participation.

Required NJ OT license, ACOTE-accredited program, CPR/BLS, and background checks; NBCOT preferred for new grads.

Qualifications

• Current New Jersey OT license.
• Graduate of an ACOTE-accredited OT program.
• NBCOT certification preferred; required for new graduates.
• Minimum of one year of clinical experience preferred; new grads considered.
• Current CPR/BLS certification.
• Clean criminal background check and fingerprinting per NJ DDD requirements.
• Reliable transportation, valid NJ driver’s license, and current auto insurance.
• Ability to pass DDD-required background checks including CRoff.

Responsibilities

• Conduct comprehensive evaluations and develop individualized treatment plans aligned with ISP and person-centered goals.
• Deliver one-on-one therapy services in home and community settings.
• Document services in compliance with DDD standards, including notes and outcomes, within required timeframes.
• Collaborate with Support Coordinators, families, caregivers, and the interdisciplinary team to ensure continuity of care.
• Provide caregiver training and home programs to reinforce goals between sessions.
• Adhere to all DDD policies and HIPAA confidentiality standards.
• Maintain accurate billing records consistent with DDD rates and authorizations.
