Job summary

Powerback Rehabilitation is seeking an Occupational Therapist to provide in-home outpatient rehabilitation for older adults. Services may be delivered in patients’ homes or in rooms at independent living facilities, enabling care in the most functional environment.

You will evaluate, develop, and execute therapy plans, supervise assistants when needed, and document progress to optimize function in daily activities. The role offers a flexible schedule and diverse home-based settings.

Qualifications

• Bachelor’s degree in OT accredited by ACOTE.
• Initial NBCOT certification.
• Valid OT license in state of practice.
• Valid driver’s license or reliable transportation to/from patient’s residence.

Responsibilities

• Provide outpatient in-home occupational therapy services for older adults.
• Evaluate and treat in the patient’s home or in an independent living facility room.
• Develop and implement individualized treatment goals and plans of care with patient and caregivers.
• Supervise OT assistants and students as required by state licensure and professional standards.
• Complete documentation and ensure timely scheduling and case conferences.
