Job summary

Powerback Rehabilitation is seeking an Occupational Therapist to deliver in-home outpatient therapy for older adults, including home visits and independent living facility settings. The role emphasizes functional goal attainment, caregiver education, and collaborative care planning.

You will evaluate, treat, and progress patients, adapt plans of care, and document outcomes while maintaining safety and quality across diverse home-based environments.

Qualifications

• Bachelor’s degree in an ACOTE-accredited Occupational Therapy program.
• Initial NBCOT registration.
• Valid OT license in practicing state.
• Valid driver’s license or reliable transportation.

Responsibilities

• Provide outpatient rehabilitation services in in-home settings for older adults.
• Coordinate and conduct OT programs to facilitate rehabilitation.
• Supervise assistants, students, and support staff as per licensure rules.
• Collaborate with rehabilitation team and physicians to plan optimal therapy.
• Document clinical notes and ensure timely billing and compliance.
• Evaluate patients and adjust plans of care to meet goals.
