Job summary

Powerback Rehabilitation To You is seeking an Occupational Therapist to provide in-home outpatient rehabilitation for older adults in various settings across the United States. You will evaluate, treat, and coordinate therapy plans in patients’ homes or in independent living facilities, emphasizing functional goals and safety.

You will supervise staff, document thoroughly, and collaborate with physicians and the rehab team to tailor the most effective treatment regimens.

Qualifications

• Must have ACOTE-accredited OT degree.
• Must be registered with National Board for Certification in Occupational Therapy (NBCOT).
• Must hold valid state OT license.
• Must have a valid driver's license or reliable transportation.

Responsibilities

• Supervise OT assistants, temporary licensees, students, and support staff per licensure rules.
• Maintain adherence to department schedule and patient treatment plans; complete clinical and billing documentation.
• Attend and contribute to patient care and staffing conferences and related meetings.
• Make referrals to other disciplines to address patient needs.
• Consult with rehabilitation team and physicians to select best therapy.
• Adapt plans of care based on individual patient needs.
• Promote a safe environment and enforce safety protocols.
• Order supplies and equipment as necessary.
• Maintain and report on equipment condition; arrange repairs as needed.
• Deliver high-quality patient service in a compassionate setting.
• Screen and evaluate patients, gather history, and apply tests to formulate diagnoses and treatment goals.
• Develop and implement treatment plans, document progress and adjust as needed.
• Provide instruction for activities of daily living and rehabilitation tasks.
• Design and fit adaptive devices and train patients in their use.
• Select appropriate activities based on patient abilities and interests.
• Evaluate progress and update goals; progress plan as needed.
• Perform routine reexaminations to modify therapy plans.
