Qualifications

• Initial registration with the NBCOT.
• Valid OT license as required in the state of practice.

Responsibilities

• Provide in-home outpatient rehabilitation for older adults.
• Plan and direct therapy with patient participation to restore function.
• Maintain timely clinical and documentation records.
• Collaborate with rehab team and other disciplines as needed.
• Ensure a safe home environment and adapt plans per patient needs.
