Job summary

21st Century Home Health Services in Lincoln, CA is seeking an Occupational Therapist to deliver skilled in-home evaluations and therapy. You will work with the interdisciplinary team to address safety, mobility, and daily living activities, helping patients live independently at home.

In this role you will manage your own caseload, plan visits with Case Manager, and provide caregiver education. A CA OT license and BLS/CPR are required, with driving to patient homes across the territory.

Qualifications

• Must hold a CA Occupational Therapist license.
• BLS/CPR certification required.
• Valid CA driver’s license required.
• Knowledge of local community resources helpful.
• Experience in home health preferred.

Responsibilities

• Evaluate patients in homes and address safety, balance, and functional needs.
• Collaborate with Case Manager and interdisciplinary team.
• Educate patients and caregivers on home safety and independence.
• Develop and implement treatment plans with measurable goals.
• Document care and participate in quality improvement initiatives.
