Job summary

21st Century Home Health Services is seeking an Occupational Therapist to evaluate and address home health needs across Santa Clara County, including Gilroy. You will assess safety, seating, pain management, and chronic conditions while partnering with Case Management and the interdisciplinary team.

The role emphasizes patient education, caregiver support, and advocacy to enable safe, productive home living. CA OT license and BLS/CPR are required, with CA driver’s license a must.

Qualifications

• CA OT license (unrestricted) required.
• BLS/CPR certification.
• Valid CA Driver's License.
• Knowledge of local community resources required.
• Knowledge of home health regulatory guidelines preferred.
• Experience in home health care preferred.
• Experience with high acuity patients is a plus.
• Experience working with an interdisciplinary team is highly desired.

Responsibilities

• Provide skilled OT services in patients' homes in partnership with the Case Manager.
• Address home safety, seating/positioning, pain management, falls prevention, and chronic disease/dementia needs.
• Educate patients and family caregivers; advocate for patients within the interdisciplinary care team.
