21st Century Home Health Services in California is seeking a Full Time Occupational Therapist to travel in Antioch and surrounding areas, delivering skilled in-home evaluations and therapy. You will work with an interdisciplinary team to promote safe, independent living for patients.

Qualifications

• CA Occupational Therapist License (unrestricted) required.
• Current BLS/CPR Certification.
• Valid CA Driver's License.
• Knowledge of local community resources.
• Experience in home health care preferred.
• Experience with high acuity patients is a plus.
• Experience working with an interdisciplinary team is highly desired.
• Case Management fundamentals is highly desired.

Responsibilities

• Evaluate and address the needs of individuals in patients' homes to enable safe, productive living.
• Address home safety, medication management, wheelchair seating and positioning, pain management, falls prevention, and chronic disease management.
• Provide caregiver training and act as educator, patient advocate, and member of the interdisciplinary care team.
