Job summary

21st Century Home Health Services is seeking a Full Time Occupational Therapist to join our field team serving the El Dorado Hills area. You will evaluate and address the needs of individuals in their homes so they can live safely and productively in their home environment.

You will collaborate with the Case Manager and an interdisciplinary care team to provide skilled OT services, address home safety, wheelchair seating and positioning, pain management, falls prevention, and caregiving support

Qualifications

• Current unrestricted CA Occupational Therapist License.
• Current BLS/CPR Certification.
• Valid CA Driver's License.
• Knowledge of local community resources is required.
• Knowledge of state and federal homecare regulatory guidelines preferred.
• Experience home health care is highly preferred.
• Experience with high acuity patients is a plus.
• Experience working with an interdisciplinary team is highly desired.
• Case Management fundamentals is highly desired.
• Self-Directed, Empathetic, Critical thinker, Outstanding interpersonal skills, Leadership, Organized.

Responsibilities

• Provide skilled OT services in patients' homes to improve safety and independence.
• Address home safety, medication management, wheelchair seating and positioning, pain management, falls prevention.
• Collaborate with Case Manager and interdisciplinary care team.
• Educate patients and families; advocate for patient needs.
• Develop and monitor care plans with the team.
